It’s last call for this year’s American Heart Association Lifestyle Change Awards.

The association is looking for “healthy makeover” stories for the Awards. Have you lost weight? Started working out? Bettered your blood pressure or cholesterol? If you have improved your health by adding physical activity and/or improving the way you eat, you may be eligible for an award.

The Lifestyle Change Awards are part of the Heart Walk campaign and the My Heart. My Life. movement, which encourages physical activity and good nutrition as part of a heart-healthy lifestyle. No change is too small, and every accomplishment is significant and can inspire others to do the same.

People can nominate friends or co-workers, or themselves. Winners will be selected by an American Heart Association committee.  They will be honored at the Nashville Heart Walk on October 6 at Vanderbilt University.

TO APPLY: Download the Lifestyle Change Awards form at www.nashvilleheartwalk.org. Send your answers to Maggie.coakley@heart.org OR mail the form back to the American Heart Association, 1818 Patterson St., Nashville, TN 37203.

The deadline is Sept. 15.
